Welcome to The Montana Department of Transportation’s Citizen Incident Notification/Report of Incident System. This service provides the public a method to submit and maintain electronic claims for vehicle and/or property damage. To file a report electronically, please login or register using the Okta Montana link on this page. Alternatively, you can print and mail the report to the Montana Department of Transportation by clicking on the "Citizen Incident Notification Form (Print Only)" link.

Disclaimer

The Department of Administration Risk Management and Tort Defense Division (RMTD) determines liability and responsibility for all claims against the Montana Department of Transportation (MDT). MDT will submit this form to RMTD. Upon receipt, RMTD will notify you by letter that your claim was received and that they will be investigating your claim. If you do not hear from them in a reasonable amount of time, please contact them at the address and phone number listed below:

Note: Our contractors are required to carry insurance. If the incident occurred on a section of highway under construction, RMTD will submit your claim directly to the contractor. The contractor is responsible for determining the responsibility and liability for claims occurring on work sites under their control.
